Hosted by Health Innovation Yorkshire & Humber, this webinar will showcase work that Hero of Health is undertaking to improve health and wellbeing and reduce pressures on primary care services.

We invite Primary Care and ICB colleagues to learn how Hero of Health has formed Integrated GP Neighbourhoods, supported by the Hero of Health App. Hero of Health’s approach has increased GP access and capacity, building disease-reversing neighborhoods that decrease GP presentations by 44% and increase patient satisfaction. Hero of Health’s approach includes:
