Who should attend?

This launch webinar is designed for the generalist multidisciplinary team working in primary care who support people living with heart failure and want access to practical, evidence-based guidance throughout the patient heart failure journey.

About the resource

Developed in partnership by The Heart Failure Academy and The Pumping Marvellous Foundation, this new booklet has been created to support the generalist primary care multidisciplinary team in delivering effective, person-centred heart failure care.

The resource follows the Heart Failure Academy’s ‘Navigating the Heart Failure Journey’ map, providing:

• Key clinical information for every stage of the heart failure journey

• Practical, bite-sized guidance for primary care teams

• Narrative insights to support personalised care conversations

• Signposting to resources that will be beneficial to support people to live well with heart failure at every stage of their journey.

The booklet has been co-designed by people living with heart failure alongside specialist heart failure multidisciplinary teams from across Yorkshire and the Humber.
