Yorkshire and Humber Patient Safety Collaborative
There are 15 Patient Safety Collaboratives in England, each hosted by a Health Innovation Network.
In Yorkshire and the Humber, our Patient Safety Collaborative (PSC) is delivered in partnership with the Improvement Academy and is led by a multi-disciplinary team of clinicians, patient safety and quality improvement experts.
For 2026 to 2027 we are focusing on the following priority areas:
- Managing Deterioration and Martha’s Rule
- Maternity and Neonatal (MatNeoSIP) – optimisation and stabilisation of the preterm infant, early recognition and management of deterioration in women and babies: And Avoiding Brain Injuries in Childbirth
- Medicines Safety (MedSIP) – Helping people with a learning disability, at risk of behaviour that challenges, avoid harm from psychotropic medicines.
- A new programme, Patient Safety Incident Response Framework (PSIRF) in General Practice, is currently being developed.
