The Heart Failure Regular Review
This interactive workshop is for healthcare professionals working in primary care to upskill in delivering a heart failure regular review
Target audience
This interactive online workshop is specifically tailored towards generalist members of the multidisciplinary team who will deliver (depending on their role) either structured medication reviews or long term condition heart failure reviews in primary care.
Why should you attend?
NHS England have released changes to the GP Contract 26/27 and there is a new indicator to support prescribing of the four pillars of therapy for people living with Heart Failure with reduced Ejection Fraction (HFrEF).
Overview
This online workshop will give you all the tools you need to increase your confidence in supporting this new QoF indicator as well as in delivering regular heart failure reviews in clinical practice.
The content will focus on a general update on heart failure, the different heart failure phenotypes, guidelines directed medical therapy for each phenotype and how to deliver a regular review as outlined by NICE NG 106.
There will also be an opportunity for Q&A to answer heart failure queries.
Aim
The aim of this study day is to support healthcare professionals to develop their knowledge and skills to provide a person centred approach to a delivering a regular heart failure review.
Learning outcomes
By the end of this online workshop, you will
- have an increased confidence in recognising the signs and symptoms of heart failure
- be able to describe the appropriate referral of people with suspected heart failure
- recognise and be able to summarise the management of different heart failure phenotypes
- apply appropriate coding for people living with heart failure
- apply a person centred, evidence-based approach to regular heart failure reviews
- describe how to identify when an advance care planning conversation is needed

Prework
Prior to attending the online workshop there is a small amount of pre-work to do: Access the Heart Failure Academy ‘Navigating the heart failure journey map’ and watch the associated videos.
